Why bother placing it in the SOURCE folder in the first place?
Just save it to the BACKUP folder and copy to FOR REVIEW (or make a second copy)

It isn't the sole culprit but it is a significant factor both in itself and in the fact that it amplifies the effects due to other causes

The old calendar control was deprecated in A2010 and doesn't work in 64-bit Access.
If you don't like the built in date picker, I recommend my Better Date Picker...

I can't answer questions specific to MySQL as I don't use it.
However, rather than drop the table then recreate it, by not link the MySQL table in Access, then empty it and repopulate it using...

Perhaps its the same control that has 'bled through so its visible on all 3 tabs.
