check boxes (1 Viewer)

I want to create a check box -
an empty text box that when the form is completed online, the person can click on a box and a cross appears in it -

like a form

I dont know how to begin

Help!

Form Controls: Have to enable the Developer Ribbon first for this ribbon to show up. Once that Ribbon is available, find the "Control" group; there you will see all kinds of form controls (check boxes, drop down lists, text boxes, etc...).
