Attached is a one-table database that runs a prepopulated Mail Merge from an Access form. In order for the document to open and the mail merge to run, you'll need to place the word document in the C: drive (no folder, just on the source drive). Place the database anywhere in your local folders. The button to run the mail merge is on the Access form upon opening the database...

My apologies. I didn't realize this, but the address used in the code for the database calls the C:\ drive as well. Both files must be placed on the C:\ drive. I just thought I would point that out.
