As Gina said--no need for a FiscalYear table. You are using a number to relate a different number. Just use the actual FiscalYear value instead of its ID and get rid of that table.

No. If you want access to produce some formatted output you are going to have to format it yourself. It can definitely run off a query to use it's data, but there's no way to import a layout
