Close form after open it (1 Viewer)

Hi,
I have a problem with one of the forms that I have in my Database.
Not every time but most of the time when I clicked on a button to open a new purchase order form, this will close the whole MS Access.
I check everywhere in the button code and the form, but nothing was found.
Anyone can help me to find the issues, please?

we can't troubleshoot if there is no sample db.

Hmm, it would help to see the code behind the button.

Or try to copy all your Objects (tables, queries, Forms, .....) to a new DB, and check the behavior of your Button.
