Query Options form (1 Viewer)

Is anyone familiar with the query options dialog form for MS Word (for MailMerge)? It's a dynamic query builder.

I would love to incorporate a form similar to this in one of my databases. I can create it myself, but I thought why re-invent the wheel.

I'll start coding it, but hopefully one of you all have already come across something similar. If so, please let me know!!

it's a v2.0! (ah, the good old days...)
as i recall, it didn't suit my needs, but you could look at the modules. might give you some ideas...
